Releases of Information
Please return these signed forms as soon as possible. You can drop these off at the office or have picked up from your home BY APPOINTMENT ONLY. Call or email any counselor for an appointment.
IHS Release of Information form – NOTE: If you would like substance abuse disorder or mental illness considered in determining if you are eligible as an individual with a disability, please check the Alcohol/ Drug Abuse or Mental Health boxes on this form.
Authorization for Release of Information – Please complete this for any school, medical clinic or other person or agency that can provide information related to your disability.
Application and Individualized Plan of Employment
Please complete all of these forms when meeting with your counselor. Once you have been determined eligible, your counselor will schedule an appointment to complete all of these together.
Application for services – We’ll ask questions to get to know you, your education, experience and lifestyle.
Financial summary – We’ll ask information about your expenses and income

Policy on NO CHARGING to vocational rehabilitation – Please read, sign and return at your convenience.
Client Assistance Program brochure – This is a program that helps people understand their rights to get vocational rehabilitation services. Please download and read at your convenience. Your counselor will discuss this during your first appointment.
